When you contact Revival
Your name, contact details, selected interest and message are saved in Revival’s restricted enquiry workspace so the team can respond. The record includes when and where you submitted it, available campaign information, your contact permission and follow-up history. Sending an enquiry does not sign you up for marketing.
Please do not submit medical history, photographs, insurance information or clinical records through these forms. Call 347-542-5895 for clinical questions.
Optional website analytics
If you allow analytics, Revival records page visits and actions such as enquiry submissions, booking-link clicks and phone-link clicks. A temporary visit identifier connects those actions within your browsing session. Names, email addresses, phone numbers and message contents are not included in analytics events. When enabled, Google Analytics receives these optional events only after you allow analytics. Google Tag Manager manages optional website tags. Advertising personalisation is disabled in our Google Analytics configuration. Google may process device, browser and network information under its own privacy practices. You can decline analytics and still use forms and booking links.
Your preference is stored on your device. With analytics declined, forms and booking links still work. An enquiry still records the page and campaign information present when you submit it.
Newsletter
Joining the newsletter records your email and marketing permission. Use the email-preferences link provided after signup to unsubscribe, or contact Revival. Signup does not mean that an email or appointment has been sent or confirmed.
Booking and security
Booking takes place in Fresha and is subject to its privacy practices. Revival records when the saved-enquiry booking link is used, but that click does not confirm an appointment. Temporary security counters limit repeated requests without retaining your raw IP address in the enquiry database.
